NYU Winthrop Hospital has provided you with a Box account to store, share, and access your files online.This is a HIPAA compliant cloud based solution, however you are solely responsible for the physical security and access to your “BOX” account. By logging into BOX you understand that the confidentially of patient information (PII/PHI) is legally mandated and of the utmost importance. You also understand that the disclosure of your logon credentials (Usernames and Passwords) to anyone under any circumstances is a violation of security and breach of hospital and patient confidentiality.
You further understand that disclosing patient information is limited only to those providing direct patient care, unless directed to do so by your supervisor. All patient information will be held in the strictest confidence.
It is your responsibility and obligation to notify Winthrop’s Information Technology Department via the IT Helpdesk (516-663-3940) immediately if I have reason to believe that the confidentiality of any of your logon credentials has been jeopardized. The Information Technology Department will immediately change the logon credential in question.
